1 child participation fee -     $25.00
2 children participation fee - $45.00
3 children participation fee - $60.00
4 or more children -             $75.00

Please fill out separate applications for each child.

The use of alcohol, tobacco, or profanity is strictly prohibited at all CASA functions. Any act of violence will not be tolerated.

CASA is an independent, volunteer organization not supported with tax dollars.
